Give your name, and reason for seeing the doctor, and then we’ll be able to give you the best amount of time available for your appointment
Please call if you are unable to keep your appointment, then some one else will be able to get in to see the doctor.
The best way to have your prescription renewed is to book an appointment with your doctor. Please don’t wait until you are out of your medicine, call 1-2 weeks before you run out. This will ensure you continue your medication without interruption. The doctor likes to see you for the renewal so that they can check how the medication is working. Perhaps they will need to give you a different dose, or a different drug. There is also the possibility that the medicine has done its job and you don’t require it any longer. On occasion the doctor will give refills of some medications. If you are not sure if you have a refill at the pharmacy you can always phone them. They will let you know.
If the doctor feels they need to discuss your test results we will send out a call back letter or call you. You are welcome to meet with your doctor to discuss all tests at any time, just phone and book an appointment.
Invalid Health Numbers
If you are from out of province, or don’t have a Health Care Number you may be expected to pay for your appointment. The fee is $48.00
To receive a copy or View your Medical Record
If you request a copy of your medical record, or a medical report you will be required to fill out a consent to release information and submit it to the front reception. We will require 10 business days to complete the request. There is a $25.00 fee for a copy of an entire medical record; there is no fee for a medical report.
The Alberta Health Care Plan pays for many of the services you receive from your doctor. However a number of services are not covered by the government plan. These are uninsured services. Some are billed to lawyers, government agencies or schools. Some are billed to you.
You or a third party such as your employer, private insurance company or school may request a letter, report, form, etc., from your physician, in addition to office materials these services require the time and expertise of your doctor and staff. As a result, you or the third party will be billed a fee to cover these costs. The reception office will discuss any fee before providing an uninsured service. Payment will be expected when you receive the service. Cash, Debit card, or Credit card is accepted. Please see reception with your payment.
These are uninsured services:
- Pre-dental anesthetic form for children and adults.
- Paternity testing
- Canada Pension Plan (CPP) forms
- Insurance Forms (e.g., AISH, Disability)
- Disabled parking plaque
- Drivers medical
- Photocopies and computer printouts ($.25/page)
